Part 1: Photo Release Form

I hereby grant 5B Anglers and its representatives the irrevocable and unrestricted right to use and publish photographs, images, or other media in which I appear for any purpose, without compensation. These photographs may be used for, but not limited to, promotional materials, social media, website, or any other marketing or publicity purposes. I acknowledge that I will not receive any compensation for the use of these photographs.

I hereby release and discharge 5B Anglers, its representatives, and any third parties acting under its authority, from any and all claims, liabilities, and demands arising out of the use of these photographs, including any claims for libel or invasion of privacy.

I am of legal age and have the right to enter into this agreement. If the participant is under 18 years of age, the parent or legal guardian must agree.

Part 2: Hold Harmless Agreement

In consideration for being allowed to participate in events organized by 5B Anglers, I agree to the following:

1. I understand that participating in Reel Kids School of Trout involves certain risks and dangers, including but not limited to physical injury, property damage, and emotional distress.

2. I hereby release, discharge, and hold harmless 5B Anglers, its officers, volunteers, and agents from any and all claims, demands, actions, or causes of action, whether in law or in equity, arising out of or related to any injury or damage that may occur as a result of participation in 5B Anglers events.

3. I understand and acknowledge that this agreement is binding not only on myself but also on my heirs, executors, administrators, and assigns.

4. I agree to indemnify and hold harmless 5B Anglers against any and all claims, liabilities, losses, damages, costs, or expenses, including attorney's fees, arising out of or related to participation in 5B Anglers events.

5. I have carefully read and fully understand the terms of this agreement, and I voluntarily sign it of my own free will.

Part 3: EVENT REFUND POLICY

Due to limited seating for events and the need to plan for materials/food/etc., cancellations must be communicated at least 48 hours before a scheduled event to give us the opportunity to fill the event. You may cancel by emailing us at hello@5banglers.org. If your cancellation is at least 48 hours in advance of the class, you will receive a full refund. If your cancellation is less than 48 hours in advance, you will receive a refund if we are able to fill your spot.
